• Required Immunizations

    All students entering the 6th grade are required to have 2 immunizations by the State of New Jersey. They are the Meningococcal Vaccine (which can be given after the student's 10th birthdate, but no later than the start of school or 2 weeks after the 11th birthdate.) and the Diptheria, Tetanus Toxoids and Pertussis Vaccine.
     
      
     
    Please note:  All incoming 6th Grade students are required to provide one of the following to the Hammarskjold School Nurse:
     
    If your child has turned 11 PRIOR to June 21st  - Please turn in all immunization records to your elementary school Nurse no later than the 21st of June.  These records must include the 2 required immunizations described above.  Not providing this information at least 2 weeks prior to orientation may delay the receipt of orientation information and your child's schedule.  Students who have turned 11, but have not provided this information will not be permitted to attend school on the first day or any day thereafter until this state mandated requirement has been met.
     
    If your child turns 11 during the summer and you have not yet turned in your immunization info to your elementary nurse - Provide a note, signed/stamped by your doctor's office documenting an upcoming appointment.  The appointment can be no later than 2 weeks past your child's birth date.  This information should be mailed,faxed or brought to Hammarskjold School as soon as you have the appointment made and must be received prior to the 6th grade orientation.  The doctor's office may also fax it to Hammarskjold at (732) 651-7135.  The immunization documentation (once your child has had their appointment) should be immediately brought/sent/faxed to Hammarskjold. It must go to the Hammarskjold Main Office where someone will be available throughout the summer.  Not providing this information at least 2 weeks prior to orientation may delay the receipt of orientation information and your child's schedule.  Students who have turned 11, but have not provided this information will not be permitted to attend school on the first day or any day thereafter until this state mandated requirement has been met.
     
    If your child turns 11 after the start of school in September and you have not yet provided immunization information to your school nurse -  As soon as possible, provide a note, signed/stamped by your doctor's office documenting an upcoming appointment.  This appointment can be no later than 2 weeks after your child's birth date.  This information should be mailed,faxed or brought to Hammarskjold School as soon as you have the appointment made and must be received prior to the 6th grade orientation.  The doctor's office may also fax it to Hammarskjold at (732) 651-7135.  The immunization documentation (once your child has had their appointment) should be immediately brought/sent/faxed to Hammarskjold.  Not providing appointment information at least 2 weeks prior to orientation may delay the receipt of orientation information and your child's schedule.  
     
    Please remember:  All students must have either their immunization completed OR proof of an upcoming appointment (within 2 weeks of the child's birthdate) prior to the 6th Grade Orientation in August.  We cannot allow students into the school without it.
     
    Not providing this necessary documentation may result in delays in receiving your child's schedule and orientation information.
