• FIFTH GRADE IMMUNIZATIONS
    New Jersey Department of Health and Senior Services require that all students
    born after January 1, 1997 and entering sixth grade are required to receive a
    Tdap and Menningococcal Vaccine. Please contact me with any questions or
    concerns.


    IMMUNIZATIONS

    New Jersey health commissioner has mandated certain immunizations for
    attendance in school.  These immunizations need to be completed before the
    beginning of school. Please contact me with any questions.


    PHYSICALS
    All children in Kindergarten and any new students into the school system
    are required to have a physical within 30 days of their registration date.

    NEW ATTENDANCE POLICY
    A note explaining a student's absence for a noncommunicable illness for a
    period of more than 5 school days must be accompanied by a physician's
    statement of the student's illness as well as a parent note.
    As always, when a student is going to be absent from school, the parent or
    guardian is responsible for calling the school and stating the reason for the
    absence.


    EMERGENCY CONTACT INFORMATION

    Please make sure your emergency contact information is complete and
    accurate.  If you change your home, work or cell number, please contact the
    school.


    MEDICATIONS
    Only prescription medicine can be dispensed during school hours.  Please have
    medication in its original container as well a medical form completed by the
    physician and parent.


    HEALTH HISTORY
    If your child's health history changes throughout the school year, such as an
    operation, hospitalization or new allergy, please let me know so I can keep
    their medical file updated.


    COLDS AND FLU INFORMATION  
    East Brunswick Public Schools is following CDC guidelines in regards to
    students with a fever. If your child has a fever(100 degrees or higher)and
    flu like symptoms such as cough and sore throat they need to stay home from
    school until they are fever free for 24 hours without the use of any
    medication.
